O'Fallon Health Center
This location functions as the single-residency continuity clinic for our program and provides a full spectrum of primary and preventive care for all age groups. Partnering with Saint Louis University School of Medicine, St. Elizabeth's Hospital and Scott Air Force Base, provides family medicine, behavioral health, obstetric and pediatric services to patients of varying backgrounds, including the military community.

Located on Scott Air Force Base in St. Clair County, Illinois, the also known as the 375 MDG, provides services for 23,000 beneficiaries and trains 42 health care professionals annually through four specialty training programs. It has affiliations with the American Hospital Association and the American Medical Association, and is accredited by the joint commission Accreditation Council for Graduate Medical Education, American Dental Association, and the College of American Pathologists.
Our residents complete the following rotations at this location:
- PGY1: Outpatient pediatrics, sports medicine
- PGY2: Dermatology, gynecology, vasectomy portion of procedures, elective options
- PGY3: Outpatient pediatrics, elective/med surg selective options

HSHS St. Elizabeth's Hospital
Completed in 2017, serves as the primary hospital for the Saint Louis University (Southwest Illinois) Family Medicine Residency. Our residents provide unopposed medical coverage for adult inpatients, emergency room medical coverage, and labor and delivery patients, along with the newborns in the nursery.
HSHS St. Elizabeth's Hospital is a member of the Hospital Sisters Health System, a multi-institutional health care system with 15 hospitals in 14 communities across Illinois and Wisconsin, and an integrated physician network. HSHS St. Elizabeth's Hospital's physicians represent more than 40 medical specialties serving patients throughout Southern Illinois.

- PGY1: FMIS, nursery, inpatient obstetrics, emergency medicine
- PGY2: FMIS, OB night float, ICU, elective options

SSM Health Saint Louis University Hospital
serves as the main teaching hospital for the Saint Louis University School of Medicine. Opened in September 2020, the 802,000 square foot hospital building and the connected SLUCare Center for Specialized Medicine complex are designed to meet the growing patient needs across Missouri and Illinois. The new hospital building replaced the previous Saint Louis University Hospital structure, which had operated in the city of St. Louis since 1933. As the largest Medicaid service provider in Missouri, the hospital and SLUCare Center for Specialized Medicine are designed to meet the growing patient needs across Missouri and Illinois.

SSM Health Cardinal Glennon Children's Hospital
is the nation's only free-standing, not-for-profit Catholic children's hospital. As a Level I Pediatric Trauma Center and Level IV NICU (the highest rating for both), this facility helps provide total care for newborn, child, and adolescent patients. Located in St. Louis, Missouri, this hospital primarily provides pediatric care for the Illinois and Missouri region, with other patients from across the world.

Touchette Regional Hospital (SIHF Healthcare)
is a general acute care facility in Centreville, IL, that has been serving impoverished Metro East communities since 1958, regardless of ability to pay. This center offers services that include a women's and newborn center, an expanded emergency department, and behavioral health and physical therapy services. Residents primarily rotate through the emergency department and inpatient psychiatric unit; however, further opportunities in the associated outpatient clinics are available.
